Modivcare Foundation

The Foundation, funded by Modivcare, is a 501(c)(3) non-profit organization and launched in 2022.

The Foundation’s mission is to help overcome everyday health disparities and inequities in the communities we serve nationwide by addressing social determinants of health (SDoH), the conditions that can greatly impact health, quality of life, and well-being.

Modivcare team members fill baskets of newborn essentials, including winter coats and car seats at Denver Health.

Modivcare’s Philadelphia team has been volunteering their time at Share Food Program to help lead the fight against food insecurity in the region. Share Food Program distributes millions of pounds of food to hundreds of thousands of neighbors in need.

How it Works

We are accepting donation and community event sponsorship requests from Modivcare team members for non-profit organizations in your community. To submit a request, please fill out the form below.

Applications can be made only by Modivcare employees.

While we read and evaluate every request, we’re unable to fund all the submissions we receive. We will let you know if your request is approved or denied. (Please allow four weeks to receive a response.)
