Our platform meets members where they are through the placement of our self-service, Smart Health Stations, and helps address the needs of members with tools designed to assess risk for chronic conditions, improve health literacy, and collect health information to inform digital navigation to community services and covered benefits.

Telehealth-enabled

  • Our newest stations are designed to support telehealth connectivity, and bring your networks closer to your members 
  • Ideal for members living in healthcare or digital deserts, where access to care may be limited by distance or connectivity issues

Seamless data integration

  • Members can easily share their profile and health information with trusted healthcare partners 
  • With consent, data collected via Modivcare’s Smart Health Stations maps to established clinical workflows and technologies
